Long Term Care
The decision that your loved one is no longer capable of living independently can be a difficult one to make. Your loved one will have a team of professionals giving them the best possible care while they are with us, including physicians, registered nurses, certified nursing assistants, social work, and therapists. This team will work with your loved one to determine how to reach their individual goals for their health and quality of life. We believe in assisting Residents in attaining and maintaining their highest level of physical and cognitive function for as long as possible.
Rehabilitative Care
We have a team of highly skilled therapists contracted from UP Rehab that work in-house here at SCMCF to help meet you or your loved ones short-term rehab goals. Our goal for our short-term rehab residents is to help them heal and strengthen enough to get back home, and we are proud to help do that. Our team works together with you and your family to develop the best plan of care to promote the ability to adapt and adjust to living as independently and safely as possible.
Memory Care Unit
Schoolcraft Medical Care Facility provides a safe, secure and homelike environment for those residents that need additional specialized care related to Alzheimer’s disease or dementia. Our staff provides meaningful daily activities to promote independence, participation, and enhance each individuals quality of life.
